Hospice Care Program
Hospice Care is the specialized comfort care provided to the terminally ill. It is a holistic approach to include physical, spiritual, and psychosocial care and support with both the client and family.

Additional programs under Hospice Care:

  • Care is provided for a wide variety of disease process in many different community settings such as homes, skilled care nursing facilities and hospitals.
  • Palliative care and pain and symptom management are priorities of our quality services.
  • Chaplain/Bereavement Program:  the Hospice chaplain’s role begins with visits where the client resides to provide spiritual support and comfort to the client and family.  This support continues in the form of bereavement and is offered to all Hospice families for 13 months.  It includes visits, newsletters and monthly group support meetings.  Our spiritual counselor is an ordained minister with many years experience with church congregations and Hospice home care.  Our chaplain respects the client’s own spiritual individuality and works in accordance to the client’s personal needs.

Types of clients who require Hospice Care:

  • Those not seeking curative treatment or treatment is no longer an option in their care
  • Clients with a prognosis of less than 6 months life expectancy and wish comfort care

Experience or qualifications of our Hospice staff:

The Hospice Team is a blend of nurses, social workers, a chaplain, home health aides and the special services of physical, occupational or speech therapies and nutrition counseling on an as needed basis, a team manager, medical director, and a wide variety of services by dedicated volunteers.

The team attends continued support meetings and inservices throughout the year to remain current with hospice disease management.
